INITIAL LOW VISION EVALUATION

Please allow two to three hours for your initial low vision evaluation. This time will include the following:

-Full History
-Detailed Assessment of Your Remaining Vision Including a Refraction and Visual Field
-Treatment/Therapy Planning

THE REHABILITATION PROCESS/OCCUPATIONAL THERAPY

Many patients who are visually impaired will benefit from Occupational Therapy. After your initial Low Vision Evaluation, your treatment play may include therapy with the Occupational Therapist.

Your initial therapy evaluation will include the following:

-Thorough Functional Assessment of Your Vision
-Goal Setting
-Treatment Planning

LOW VISION REHABILITATION
